Read more about senior care →A note on age transitions. When a child on a family membership turns 18, their rate updates to the $90 adult fee. When any member turns 65, the rate updates to the $120 senior fee. We reach out to the account holder before either change so nobody is surprised.
Medicare works a little differently.
Our providers have opted out of Medicare, as most direct primary care practices must. Your Medicare coverage stays active for everything else, though a few Medicare-covered preventive services need a participating provider, so most senior members keep one in the picture.

HSA dollars now cover Prime Direct Health.
As of January 2026, federal rules allow Health Savings Account funds to pay your direct primary care membership pre-tax. Depending on your tax bracket, that's an effective 20 to 35 percent discount. If you have an HSA and aren't using it for primary care, you're leaving money on the table.
We cap our practice size.
Each Prime Direct Health provider cares for 400 to 600 patients, compared to 2,000 to 2,500 at a typical practice. That's how we keep visits long, scheduling fast, and your provider familiar with your history. When we hit capacity, we pause new enrollment until the next location opens.
Small problems get expensive.
A $7 lipid panel now is a small investment in catching things early. Waiting is what makes problems expensive. The patients who join Prime Direct Health catch things sooner because the cost and friction of a visit stops being the reason to put it off.

- Same-day or next-day visits when you're sick. Call or text and we'll get you on the schedule.
- Telehealth visits when a conversation is all that’s needed.
- Annual physicals and age-appropriate screenings.
- Chronic condition management (blood pressure, diabetes, cholesterol, thyroid).
- In-office procedures (EKG, spirometry, blood glucose check, pregnancy test, urinalysis with UTI management, COVID/Strep/Flu swabs, skin tag and mole removal, ear lavage, ingrown toenail care, and cryotherapy for warts and skin lesions).
- Mental health support. Anxiety, depression, and persistent low-mood get handled here, no different from anything else. Medication when it’s the right call. For complex needs (mood disorders, ADHD, controlled-substance management) we refer to a trusted psychiatrist and stay involved as your primary care home.
- Direct phone and text access to your provider.
- Coordination of specialist referrals and imaging.
What's not included
Prime Direct Health is primary care. Specialist care, hospitalization, emergency care, and long-term chronic pain management happen outside the membership. Most members pair Prime Direct Health with a high-deductible insurance plan as a safety net for hospital admissions, surgery, and other large medical expenses, subject to their specific plan's terms. If you're shopping catastrophic plans, note that most of them won't cover routine specialist visits, so factor that into your coverage planning.
- Specialist visits (cardiology, dermatology, GI, and others).
- Surgery and hospital admissions.
- Emergency room visits.
- Chronic pain management. It's its own specialty and belongs with a provider who does it full time.
What labs and medications cost.
Open book. We buy near cost and pass the price through.
Common labs like CBC, lipid panel, A1c, thyroid, and vitamin D typically run $5 to $25 cash at Prime Direct Health. More involved work runs higher: a comprehensive hormone panel for men is around $65, and for women around $80. Comparable lab work billed through insurance usually runs $50 to $200 before any deductible kicks in.
Common generic medications (statins, metformin, lisinopril, levothyroxine, and the like) typically run a few dollars a month.
An in-house pharmacy is coming. Once it opens, common medications can be picked up here at wholesale prices instead of at an outside pharmacy.

An HSA, or Health Savings Account, lets you set aside pre-tax money for medical expenses. It comes with some high-deductible insurance plans.
Starting January 2026, federal law explicitly allows direct primary care memberships to be paid through your HSA, when paired with a high-deductible insurance plan. The cap is $150 per individual or $300 per family per month, and Prime Direct Health's $90 adult and $120 senior tiers fall well under that. Your full membership can be paid with pre-tax HSA dollars.
Depending on your tax bracket, that effectively reduces the cost by 20 to 35%. Don't have an HSA or aren't on an HDHP? You pay normally. The membership doesn't require it.

Where urgent care still fits.
An honest answer to a question we get a lot: do I still need urgent care?
Prime Direct Health is open Monday through Thursday and Friday mornings. Urgent care is open evenings and weekends. Both get used.
What changes when you have Prime Direct Health: the daytime urgent-care visit usually goes away. The strep test, the rash, the blood pressure check, the "is this thing serious?" run. Those become a same-day or next-day visit with your provider, included in your membership.
What stays the same: an after-hours injury, a sick kid at 9 PM, a Saturday morning flu. Urgent care is still the right call for those moments.

I already have insurance. Why would I pay for this too?
Insurance is where you want coverage for the expensive, rare events: ER visits, hospital stays, specialty care. For day-to-day primary care, insurance often gets in the way. High deductibles mean you’re paying out of pocket anyway for visits and labs. Most Prime Direct Health members pair the membership with a high-deductible plan for those big medical events and use the membership for everything else.
$90 a month sounds like a lot for primary care.
Compare it to one urgent care visit, which usually runs $150. Or one ER trip you could’ve avoided, which can run $1,500 or more. For unlimited primary care, same-day or next-day access, and a provider who knows you, the math usually works out, especially if you’ve been avoiding care because of cost.
I hardly ever go to the doctor. Why pay monthly?
