What are Deedee's credentials?
Deedee Keeler is a board-certified Family Nurse Practitioner (FNP-C). In Arizona, nurse practitioners practice independently, so she diagnoses, prescribes, orders labs and imaging, and manages chronic conditions, the same scope a primary care physician covers for the conditions Prime Direct Health treats. Family nurse practitioner means she's licensed and trained to care for patients across the full age range, from kids through seniors. Before family practice she spent a decade in the U.S. Navy and seven years as a Banner ER nurse, and she received the DAISY Award for Extraordinary Nurses in 2022.

$90 a month for adults. $120 a month for seniors 65 and over. Family pricing applies to any combination of household members: $90 for the first, $60 for the second, and $30 for each additional. Members 65 and up stay on the $120 senior rate. A one-time single visit without a membership is $150.
What does the membership cover?
Unlimited primary care visits, same-day or next-day access, in-office procedures, anxiety and depression support, telehealth, and direct phone and text access to your provider. Labs are billed separately at near-cost prices. Medications and IV hydration are billed separately too.
Are there hidden fees or annual add-ons?
No. The monthly fee is the monthly fee. Labs and medications cost what they cost (we show you before we order anything), and there are no annual fees, initial registration fees, or surprise charges.
Can I pause or skip a month?
Membership is month-to-month, and there's no pause option. To cancel, we ask for 30 days' notice. Your membership renews on the 1st of each month, so if your request comes in less than 30 days before the next renewal, it renews once more and you stay covered through the end of that month. Cancellation is free; if you rejoin later, returning members pay a one-time $100 re-enrollment fee.

No. You can join Prime Direct Health for yourself and keep your kids on whatever insurance they're on. Plenty of our members start solo and add family members later if it makes sense. When you do, the second household member is $60 a month and each additional member is $30, on top of the $90 primary plan.
Do you see kids?
Yes. Prime Direct Health sees patients of every age, including kids. We can handle sick visits, well checks, sports physicals, school physicals, and ongoing care for kids the same way we do for adults. Family pricing makes it straightforward to bring everyone under one membership: $90 for the first, $60 for the second, and $30 for each additional household member.

Do you take insurance?
No, and that's the point. Prime Direct Health doesn't bill insurance. Members pay a flat monthly fee directly to the practice. Most members keep insurance on the side, usually a high-deductible or catastrophic plan, to handle hospital and specialist care.
Do I still need health insurance?
Yes, for the things Prime Direct Health doesn't cover. ER visits, hospital stays, surgery, specialist care, imaging. A high-deductible or catastrophic insurance plan paired with Prime Direct Health is the most common setup.
Can I use my Medicare at Prime Direct Health?
Our Prime Direct Health providers have opted out of Medicare, which is required for most direct primary care practices. Medicare won't be billed for the care you get here. You'll pay our flat $120 senior membership directly. There's a real wrinkle to know about: the Annual Wellness Visit and a few Medicare-required preventive services have to happen with a Medicare-participating provider. Most senior Prime Direct Health members keep a separate Medicare PCP in the picture for those specific items.

Almost always, yes. Prime Direct Health membership doesn't conflict with employer insurance. You keep the employer plan for the things it's good at and use Prime Direct Health for primary care. If your employer offers an HDHP with an HSA, Prime Direct Health membership is HSA-eligible.
What's an HSA?
A Health Savings Account. Pre-tax money you set aside for qualifying medical expenses. It comes with some high-deductible insurance plans and rolls over year to year.
Can I use my HSA to pay for Prime Direct Health?
Yes. Starting January 2026, federal law explicitly allows direct primary care memberships to be paid through an HSA when paired with a high-deductible insurance plan. The cap is $150 per individual or $300 per family per month, which Prime Direct Health's tiers fall well under.
How much does that save me?
Depending on your tax bracket, paying with pre-tax HSA dollars effectively reduces the cost by 20 to 35%.
What if I don't have an HSA?
Pay normally. The membership doesn't require an HSA. The 2026 rule is a tax advantage for people who happen to have one, not a requirement for joining.
What's included in my membership?
Annual physicals, sick visits, chronic condition management, in-office procedures (EKG, spirometry, blood glucose check, pregnancy test, urinalysis with UTI management, COVID/Strep/Flu swabs, skin tag and mole removal, ear lavage, ingrown toenail care, and cryotherapy for warts and skin lesions), mental health support for anxiety and depression (complex needs coordinated with referred psychiatric providers), telehealth visits, and direct provider access.

Specialist visits, surgery, hospital admissions, ER visits, and imaging. Those fall outside your Prime Direct Health membership. We coordinate the referral, send orders for imaging that gets done off-site, and stay in the loop with your care.
What about labs?
Labs are billed separately at near-cost prices. Common lab work runs $5 to $25 each at Prime Direct Health, compared to $50 to $200 through insurance. We show you the price before we order anything.
What about medications?
An in-house pharmacy is coming. Once it opens you will be able to pick up common medications right here at wholesale prices. Until then, prescriptions work like anywhere else. We prescribe, you fill at your pharmacy. Many common generics run a few dollars a month at cash-pay prices, often cheaper than running through insurance. Specialty medications are billed separately by the pharmacy, and we'll walk through what to expect during your visit.
What about MRIs, CT scans, and other imaging?
Imaging happens through outside imaging centers. We coordinate the referral, help you find affordable cash-pay options when relevant, and review the results with you. The imaging itself is billed by the imaging center, not by Prime Direct Health.
What happens at a free meet and greet?
A free 20-minute conversation with Deedee in the Gilbert office. There's no exam, no paperwork to sign, and no pressure. You'll get a tour, talk through your health situation, what's been bothering you, what you're hoping for, and how Prime Direct Health works. You're welcome to bring a spouse or partner. We'll answer every question you have, including the ones you've been afraid to ask. If it's a fit, you can sign up that day and we'll move into onboarding from there. If it's not, we'll part as friends and hope to see you back if anything changes.

The meet and greet is a free conversation to see whether Prime Direct Health is a fit. Onboarding is the practical setup after you decide to join. We collect a medication list, payment information for your membership, and your insurance card if you have one. We don't bill your insurance, but we keep it on file so we can coordinate with hospitals, specialists, and any labs or imaging that route through your insurance. Some patients handle onboarding online before they walk in for their first visit. Others do it in the office, especially when they sign up right at the end of a meet and greet. Either way, the goal is to get the paperwork settled before your first real appointment so the visit itself stays focused on you.
How long is the first real appointment after I join?
Visits at Prime Direct Health run 30 to 60 minutes. Your first appointment after joining will fall in that range and may run longer if there's a lot to cover from your history.
What should I bring?
A list of medications you're taking, any recent lab results if you have them, and any questions you've been holding onto. No insurance card needed.
Can my whole family join at once?
Yes. The first member is $90, the second is $60, and each additional household member is $30, with seniors 65+ on the $120 senior rate. We set up a single account for the whole household and walk through the specifics on a meet & greet.
How do I cancel?
Fill out the short cancellation form. We ask for it in writing so the request is documented and nothing falls through the cracks. We require 30 days' notice, and since membership renews on the 1st of each month, a request made less than 30 days before your next renewal means one more renewal before it ends, so you stay covered through the end of that month. The form asks for your reason, because the answer helps us improve the practice, but your reason never affects whether or how quickly we process the cancellation.

Can I rejoin later?
Yes. Returning members pay a one-time $100 re-enrollment fee. New members join free. The re-enrollment fee is there because membership works best as a real relationship, and the structure keeps people from cycling in and out of the practice based on what they happen to need that month.

Yes. Both are part of Prime Direct Health's weight management program. Visits are included in your membership. Medication is billed separately at transparent prices.
How is this different from telehealth weight loss?
Same provider every visit, real lab monitoring, in-person availability when you need it, and the relationship doesn't end when you hit your goal. Telehealth services typically rotate providers and skip the monitoring.

Whether GLP-1 medication is the right step is a clinical conversation, not a price quote. We start by understanding your health history, blood work, and goals. If medication is the right call, we'll walk through the options (formulation, dosing, what's available right now) at your visit. Medication is billed separately by the pharmacy, not by Prime Direct Health.
Do you offer testosterone replacement therapy (TRT) for men?
Yes. TRT is active. Full evaluation, lab work, dosing, and ongoing monitoring all happen inside your primary care, by the same provider managing the rest of your health.

Hormone levels, related blood work (lipids, hematocrit, prostate markers for men), and the rest of your health that hormones interact with: blood pressure, mood, metabolism, medication list. The same provider sees all of it.
How do I reach you outside business hours?
Members have direct contact information for urgent issues. Non-urgent messages get a response on the next business day. For emergencies, call 911.
What if I'm out of town?
Telehealth visits work anywhere. If something serious comes up while traveling, we can usually triage by phone and decide whether you need urgent care locally or whether it can wait until you're home.
What if I have an actual emergency?
Call 911 or go to the nearest ER. Prime Direct Health is primary care. We're not equipped for emergencies. After you're stable, let us know and we'll coordinate the follow-up.
Who owns Prime Direct Health?
Prime Direct Health is owned by Dr. Sunny and Dr. Mark Schlotterback (50/50) under Prime Medical Group. Both are practicing physicians, but their work for Prime Direct Health is on the business side. Day-to-day care at Prime Direct Health is led by Deedee Keeler, FNP-C, our practicing nurse practitioner.
